COVID-19 and TeleHealth Services

During times of anxiety and uncertainty, we take our role as behavioral health care providers with the utmost seriousness. We are prepared to keep our clients, staff, and community as safe as possible while also doing our best to minimize disruptions to care.

All TeleHealth clients must fill out this consent form so that we may use these services and also process your co-pay remotely to simplify your visit. You may email your signed form to synapse@fulbrightrehab.com or please call (972) 250-1705 for additional options.
